Abstract

Suppurative hidradenitis (HS) is a debilitating, immune-mediated inflammatory disease, whose estimated prevalence is 0.41% in the Brazilian population. It results from the chronic occlusion of the pilosebaceous follicle in the areas of apocrine glands and manifests itself through nodules, abscesses, fibrosis and fistulous pathways. This paper aims to report the case of a patient with atypical clinical manifestations, of a follicular pattern, in which, through history and a detailed physical examination, he was diagnosed with HS. A retrospective analysis was carried out based on the medical records of the patient seen in July to November 2020 at the outpatient clinic of the Clinical Dermatology service of the Associação Pele Saudável - Faculdades BWS. Some diagnoses were closed, such as folliculitis dissecans and moderate inflammatory acne. However, a syndromic disease became evident: HS. The therapy instituted was through drug therapy with Acitretin and infiltrations with corticosteroids. The understanding of HS and its clinical characteristics allowed an adequate diagnostic conclusion and can directly influence the systemic treatment decision, avoiding underdiagnosis and its complications.
